Diagnosis of hypertension in childhood requires repeated BP measurements

One needs to confirm presence of hypertension based on three blood pressure measurements at separate clinical visits.

Normative BP percentiles are based upon data on gender, age, height, and blood pressure measurements from the National Health and Nutrition Examination Survey and other population–based studies.

In a study initial BP measurement was normal (below the 90th percentile), pre–hypertensive (systolic or diastolic BP between the 90th or 95th percentile) and hypertensive (systolic or diastolic BP ≥95th percentile) in 82, 13, and 5 percent of children.

At follow–up, subsequent hypertensive measurements were observed in only 4 percent of the 10,848 children who had initial hypertensive values. In the cohort, the overall prevalence of hypertension was 0.3 percent.

  • Despite past reports suggesting a link between marijuana use and increased stroke risk, a recent study presented at the American Academy of Neurology (AAN) 66th Annual Meeting has shown a statistically significant inverse relationship between marijuana use more than a year ago and ischemic stroke risk in young people.
  • In another new revelation from the 66th AAN Annual Meeting, researchers have found both pro– and anti–inflammatory epigenetic factors in the intestinal microbiota of patients with multiple sclerosis (MS) that may be implicated in the disease pathogenesis.
  • The US FDA’s Nonprescription Drugs Advisory Committee has voted against recommending montelukast to be approved as an over–the–counter (OTC) drug for the treatment of allergic rhinitis (AR).
  • A study presented at the American Pain Society (APS) 33rd Annual Scientific Meeting has stated that a novel extended–release formulation of hydrocodone having antiabuse properties provides safe and long–lasting pain relief to patients with chronic pain, including those who have used opioids before and those who are opioid–naive.
  • A study presented at the American Congress of Obstetricians and Gynecologists (ACOG) 2014 Annual Clinical Meeting has pointed that women who require reoperation for recurrent prolapse could possibly benefit from vaginal mesh repair. Mesh erosion was more common in women who had not previously undergone vaginal prolapse surgery as compared to those who had (10.6% vs 2.8%).

What is the role of monoclonal antibodies in Rabies?

Monoclonal antibodies against rabies virus have been widely used in the diagnosis and immunological analysis of rabies. Human monoclonal antibodies to rabies virus G protein are also expected to be used as a replacement for rabies immunoglobulin (RIG) in the post–exposure treatment of rabies. In 1978, Wiktor reported the preparation of rabies virus monoclonal antibodies. Since then, rabies virus monoclonal antibody (mAb) technology has been more and more widely used in basic research and diagnosis of rabies.

  • Long–term results from the Study of Heart and Renal Protection (SHARP) have shown that for patients with chronic kidney disease (CKD), lowering LDL cholesterol with a combination of simvastatin and ezetimibe may not curb the progression of kidney disease to end–stage renal disease (ESRD). The findings are published May 1 in the Journal of the American Society of Nephrology.
  • The American Heart Association/American Stroke Association (AHA/ASA) has issued new guidelines on the secondary prevention of stroke. While the guidelines stress upon the importance of blood pressure, cholesterol, weight, and exercise, they also include some important new recommendations like screening stroke and transient ischemic attack (TIA) survivors for diabetes and obesity, monitoring for atrial fibrillation (AF) among patients with a stroke of unknown cause, use of new oral anticoagulants in specific situations, etc.

  • An international team of researchers has developed a blood test that will assist doctors in quickly identifying most active tuberculosis infections in children. Based on a genome–wide analysis of RNA transcripts in the children’s blood, researchers found a 51–transcript signature that distinguished TB from other diseases, and 42 transcripts that could differentiate it from latent infection.
  • A study published online April 3 in the Journal of the Pediatric Infectious Diseases Society has reported that treatment of cutaneous leishmaniasis with systemic meglumine antimoniate is well tolerated in children and the rate of adverse events is similar to that seen in adults.

Why do we place our hands over the flame?

Flame is the "flame" of true knowledge. At the end of any aarti, we place our hands over the flame and then touch our eyes and the top of the head. It means "May the light that illuminated the Lord light up my vision; May my vision be divine and my thoughts noble and beautiful."

The metaphysical implication of aarti extends further. The sun, moon, stars, lightning and fire are the natural sources of light. The Lord is the source of these wondrous phenomena of the universe. It is due to Him alone that everything exists.

As we light up the Lord with the flame of the aarti, we turn our attention to the very source of all light which symbolizes knowledge and life. Also, the Sun is the presiding deity of the intellect, the moon, that of the mind, and fire, that of speech. The Lord is the supreme consciousness that illuminates all of them. Without Him, the intellect cannot think, the mind cannot feel and the tongue cannot speak. The Lord is beyond the mind, intellect and speech.

How can these finite entities illuminate the Lord? Therefore, as we perform the aarti we chant:

Na tatra suryo bhaati na chandra taarakam, Nemaa vidyuto bhaanti kutoyamagnib

Tameva bhaantam anubhaati sarvam, Tasya bhasa sarvam idam vibhaati

"He is there where the sun does not shine, nor the moon, stars and lightning. Then what to talk of this small flame (in my hand), everything (in the universe) shines only after the Lord, and by His light alone are we all illumined"

In our spiritual journey, even as we serve the guru and society, we should willingly sacrifice ourselves and all we have, to spread the "perfume" of love to all.

We often wait a long while to see the illuminated Lord. But, when the aarti is actually performed, our eyes close automatically as if to look within. This is to signify that each of us is a temple of the Lord.

Add fiber to the diet only slowly

Fiber is a plant substance that is required for a healthy diet. Lots of fiber is needed each day to help reduce the risk of heart disease, improve digestion, prevent constipation and maintain a healthy body weight.

Fiber can be found in fruits, whole grains and vegetables. Most adults should eat at least 20 to 35 grams of fiber every day; though the doctors say most people only eat about half as much. It’s best to slowly increase the fiber in your diet instead of piling it on all at once. A sudden increase in fiber intake can cause abdominal discomfort.

Fiber intake should be at least 14 grams per 1000 calories daily; higher fiber intake may improve glycemic control. Saturated fat should be less than 7 percent of calories and there should be minimal trans fat. Total cholesterol should be less than 200 mg daily.

There is an average fall of 1.2/1.3 mmHg blood pressure with average 10 gram intake of fiber.

Certain soluble fibers (psyllium, pectin, wheat dextrin and oat products) reduce bad LDL cholesterol. Every gram increase in soluble fiber reduces LDL cholesterol by an average of 2.2 mg/dL. The message is incorporation of greater amounts of fiber, in which carbohydrate is derived from unprocessed whole foods.

The Greatest Gift is Love

A woman came out of her house and saw three old men with long white beards sitting in her front yard. She did not recognize them.

She said, "I don’t think I know you, but you must be hungry. Please come in and have something to eat." "Is the man of the house home?" they asked. "No", she said. "He’s out." "Then we cannot come in", they replied. In the evening when her husband came home, she told him what had happened. "Go tell them I am home and invite them in!" The woman went out and invited the men in. "We do not go into a House together," they replied.

"Why is that?" she wanted to know. One of the old men explained: "His name is Wealth," he said pointing to one of his friends, and said pointing to another one, "He is Success, and I am Love." Then he added, "Now go in and discuss with your husband which one of us you want in your home."

The woman went in and told her husband what was said. Her husband was overjoyed. "How nice!!" he said. "Since that is the case, let us invite Wealth. Let him come and fill our home with wealth!" His wife disagreed. "My dear, why don’t we invite Success?"

Their daughter–in–law was listening from the other corner of the house. She jumped in with her own suggestion: "Would it not be better to invite Love? Our home will then be filled with love!" "Let us heed our daughter–in–law’s advice," said the husband to his wife. "Go out and invite Love to be our guest."

The woman went out and asked the 3 old men, "Which one of you is Love? Please come in and be our guest." Love got up and started walking toward the house. The other two also got up and followed him. Surprised, the lady asked Wealth and Success: "I only invited Love, Why are you coming in?"

The old men replied together: "If you had invited Wealth or Success, the other two of us would’ve stayed out, but since you invited Love, wherever He goes, we go with him. Wherever there is Love, there is also Wealth and Success!"

Kidney stone of less than 5mm size needs no treatment

According to guidelines for management of kidney stones, a stone less than 5mm in size in the kidney passage may pass out in the next four weeks and so does not need treatment, said Padma Shri, Dr. B C Roy National Awardee & DST National Science Communication Awardee, Dr. K K Aggarwal, President Heart Care Foundation of India and Sr National Vice President Indian Medical Association.

A stone between 5 to 10 mm has only 20% chances of passing spontaneously. A stone, which is more than 10mm in size, will invariably require a non–medical intervention.

The new avenues in stone management include a trial by drugs, which help in expulsion of the stone by relaxing the smooth muscle. The drugs used are Nifedipine (calcium channel blocker), alpha–blockers and steroids.

Up till now it was thought that the best investigation for renal stone is intravenous pyelography. But now it has been proven beyond doubt that when a patient presents with kidney pain and a renal stone is suspected the undisputed investigation of choice is unenhanced CT scan of the abdomen. As per the guidelines, open removal of stones is no more recommended. Lithotripsy, PCNL and ureteroscopy have practically taken over from the open surgical procedures.

North India is considered a stone belt, with maximum cases of kidney stones in India.
