 IV Fish oil may treat sepsis

Trials involving fish oil given intravenously to patients in ICU suggest that it reduces inflammatory chemicals and results in shorter hospital stays. In a new randomized controlled trial from the University of Southampton, Professor of Nutritional Immunology, Philip Calder, found that oil improves the way patient's lungs were working through improved gas exchange whilst also reducing inflammatory chemicals. The study is published in BioMed Central's open access journal Critical Care.


Twenty-three patients were studied. All the patients had systemic inflammatory response syndrome, or sepsis, and were on ventilators. The fish oil replaced another type of fat they would have been given as part of normal intravenous nutrition.The 13 patients in the fish oil group had lower levels of inflammatory agents in their blood, were able to achieve better lung function and left hospital earlier than the 10 patients who received traditional nutrition.
Traditional solutions use soybean oil, which is rich in omega-6 acids and may actually make inflammation worse. Fish oil, on the other hand, is rich in omega-3 fatty acids that work to reduce inflammatory responses.

FDA shifts stance on safety of BPA
In a shift of position, the US FSA is expressing concerns about possible health risks from bisphenol-A, or BPA, a widely used component of plastic bottles and food packaging that it declared safe in 2008. The FDA said it has "some concern about the potential effects of BPA on the brain, behavior and prostate gland of fetuses, infants and children."

Doctors seek to ease needle phobias
At least 10% of adults are afraid of needles, and scientists have some evidence that painful early experiences -- including circumcisions performed without anesthetic -- can prime the nervous system to experience pain more intensely in the future.

CDC to issue circumcision recommendations
CDC has decided "to issue recommendations for the first time about whether newborn boys and possibly even adult men should undergo" circumcision. The evidence of potential benefits of circumcision "has gotten much stronger" recently. Circumcision has been shown to reduce "the risk for urinary tract infections among infants," and "studies indicate that circumcision cuts the chances of adult men's getting penile cancer and becoming infected with a variety of sexually transmitted diseases." The procedure may also reduce "the risk for getting the AIDS virus."

US Task Force issues new guidelines to fight childhood obesity
The US Preventive Services Task Force issued new guidelines that say 'school-aged youngsters and teens should be screened for obesity and sent to intensive behavior treatment if they need to lose weight’, a move that could transform how doctors deal with overweight children.

Appendicitis may be "a viral disease."
Perforated, or ruptured, appendicitis is a different disease from non-perforating appendicitis. In other words, some inflamed appendixes won't burst, no matter how long you wait to remove them. Appendicitis may be a viral disease. (Texas University)

Radiofrequency ablation may offer alternative to hysterectomy for uterine fibroids.
Radiofrequency ablation, offers an alternative to a hysterectomy for women with fibroids. The procedure, which involves a needle-like device that is inserted through the patient's abdomen into the fibroid and heated using low frequency electrical current, is currently being tested in phase III clinical trials for uterine fibroids. Meanwhile, other alternatives include anti-inflammatory drugs, hormone blockers, myomectomy, uterine artery embolization and high-frequency ultrasound.

Testosterone, HGH treatments gaining popularity for anti-aging uses
Increasing numbers of men...are turning to testosterone and human growth hormone (HGH) in an effort to look younger, be healthier, and feel more vital later in life. The National Institute on Aging says more research is needed on testosterone treatment for most issues, and the institute "cautions against using growth hormone therapy for anti-aging purposes."

Higher prescribed doses of opioids may increase overdose risk
A paper published in the Annals of Internal Medicine reveals that more doctors are prescribing oxycodone, morphine, and other opioid painkillers for back pain, arthritis, and headache.

Unnecessary C-sections have reached epidemic proportions
A WHO study has determined that nearly half of all births in China are delivered by cesarean section. The procedures are largely unnecessary, costlier than natural births, and raise the risk of complications for the mother.

Angiotensin receptor blockers may reduce dementia
According to a study published in the British Medical Journal, angiotensin receptor blockers may also reduce the risk of developing Alzheimer's disease.

Dutogliptin may improve glycemic control in type 2 diabetes According to a study in Diabetes, Obesity and Metabolism, dutogliptin treatment improves glycemic control in patients with type 2 diabetes who are poorly controlled on metformin and/or a thiazolidinedione.

BPA associated with increased risk of heart disease
UK researchers have shown that bisphenol A can cause developmental problems in children. Studies have also linked high levels of BPA in the body to an increased risk of heart disease.

Dr. A.K. Bhalla, Senior Consultant Nephrologist, is a specialist in Peritoneal Dialysis and has established the peritoneal dialysis services as a modality of Renal Replacement Therapy in India at Sir Ganga Ram Hospital. He has been recognized as pioneer in the field. Dr. A.K. Bhalla is a Founder Member of Peritoneal Dialysis Society of India.

Since the time he joined the department at Sir Ganga Ram Hospital, the kidney transplant services have just started. Since then the department has performed over 1500 kidney transplants. The department is recognized as leading center for kidney transplantation in India.

Dr. A.K. Bhalla is overlooking the Dialysis Unit at Sir Ganga Ram Hospital, which is one of the largest and ultra modern units of the country providing the state of art dialysis procedures. Presently around 1500 hemodialysis are performed every month. Dr A.K. Bhalla is actively involved in vascular access surgery for hemodialysis and has innovated new techniques, which were presented at Japanese Society of Dialysis therapy at Sapporo, Japan. He has performed around 5000 vascular access surgeries so far. 

The department is established as a referral tertiary care center for Delhi and adjoining States and has earned excellent reputation.Dr. A.K. Bhalla's main area of interest has been Peritoneal Dialysis (CAPD & other modalities). In 1989, he started CAPD modality at Sir Ganga Ram Hospital. In 1994 he visited Singapore General Hospital for highly specialized training in Peritoneal Dialysis.

He established the peritoneal dialysis services at Sir Ganga Ram Hospital and so far he has around 1200 patients on continuous Peritoneal Dialysis, which is largest in the country. Every year he has been organizing "CAPD Patient's Meet" as Programme Director.

What are the diagnostic tests to identify species of the malarial parasite?
In areas where two or more species of malaria parasites are common, only a parasitological method will permit a species diagnosis. Where mono-infection with P. vivax is common and microscopy is not available, it is recommended that a combination RDT which contains a pan-malarial antigen is used. Alternatively, rapid diagnostic tests (RDTs) specific for falciparum malaria can be used. Treatment for vivax malaria is given only to cases with a negative test result but a high clinical suspicion of malaria. In areas where P. vivax, P. malaria or P. ovale occur almost always as a coinfection with P. falciparum, an RDT detecting P. falciparum alone is sufficient. Antirelapse treatment with primaquine should only be given to cases with confirmed diagnosis of vivax malaria.

Dr Bad: Take any NSAID.
Dr Good: You cannot take painkillers for long time.
Lesson: Nonselective NSAIDs should be used with caution, following a stepped-care approach in patients who have known cardiovascular disease or are at increased cardiovascular risk. NSAIDs should be used in the lowest effective dose and the shortest duration necessary for the given indication.

Situation: An adult undergoing bronchoscopic biopsy developed infective endocarditis (IE).
Reaction: Oh my God! Why was IE prophylaxis not given?
Make sure that all procedures of the respiratory tract that involve incision or biopsy of the respiratory mucosa include IE prophylaxis.
