New Epilepsy Definition Adopted

The International League Against Epilepsy (ILAE) has expanded the definition of epilepsy to incorporate a single unprovoked seizure with a probability of future seizures. The new definition also stipulates that epilepsy can be considered "resolved" if a patient has been seizure–free for 10 years, with no seizure medicines for the last 5 years. The previous definition stipulated that a patient needs to have 2 unprovoked seizures more than 24 hours apart.

Dr Robert Fisher at Stanford University School of Medicine, California published his data online April 14 and appearing in the April issue of Epilepsia, that describes the new epilepsy definition.

According to the new definition, epilepsy is a disease of the brain defined by any of the following conditions:

  1. At least 2 unprovoked (or reflex) seizures occurring more than 24 hours apart
  2. One unprovoked (or reflex) seizure and a probability of further seizures similar to the general recurrence risk (at least 60%) after 2 unprovoked seizures, occurring over the next 10 years; or
  3. Diagnosis of an epilepsy syndrome.

Comments

  1. The new definition incorporates "reflex" seizures, for example, seizures that occur upon exposure to flashing lights.
  2. The new definition includes "diagnosis of an epilepsy syndrome," or a collection of medical signs and symptoms, for example, benign epilepsy with centro–temporal spikes in kids.
  3. As before, a person could have definite epilepsy and the decision could be made not to treat; maybe the seizures are too minor or the treatment is worse than the risk of another seizure in the eye of the patient.
  4. Inclusion of more patients into the definition of epilepsy could affect estimates of incidence and prevalence.

A study published online in the Journal of Clinical Oncology has revealed that women who survived childhood cancers treated with chest radiotherapy had a 20–times increased risk of developing breast cancer than women in the general population. The magnitude of the risk was 43.6 times greater in women who received a lower radiation dose to a large volume (whole lung).

The FDA is requiring the addition of a Warning to the drug labels of injectable corticosteroids to describe the risk of serious adverse events, including loss of vision, stroke, paralysis, and death, after injection into the epidural space.

Drug treatment and weight loss can restore lost vision, NIH–funded study shows

Acetazolamide when added to a weight loss plan, can improve vision for women with a disorder called idiopathic intracranial hypertension (IIH), according to a study funded by the National Institutes of Health.

IIH, also called pseudotumor cerebri, predominantly affects overweight women of reproductive age. An estimated 100,000 Americans have it, and the number is rising with the obesity epidemic. The most common symptoms are headaches and visual problems, including blind spots, poor side vision, double vision and temporary episodes of blindness. About 5–10 percent of women with IIH experience disabling vision loss.

The trial was funded by NIH’s National Eye Institute, and coordinated by the Neuro–Ophthalmology Research Disease Investigator Consortium (NORDIC).

  • A latest French study has warned that marijuana use may increase the risk of cardiovascular events, especially in younger adults, with fatal consequences. The types of cardiovascular complications may range from acute coronary syndromes, heart–rate disorders to peripheral vascular events, and cerebral–artery events.
  • Maintaining warfarin anticoagulation appears safer than interrupting it for catheter–ablation procedures for atrial fibrillation, according to a randomized study involving >1500 patients at increased risk for thromboembolic stroke. The study authors noted that patients without periprocedural warfarin, bridged with low–molecular weight heparin in standard fashion, had >10–fold increased odds of ischemic stroke or transient ischemic attack (TIA) in the 48 hours after ablation, compared with those on uninterrupted warfarin.

  • A meta–analysis of 9 studies from 6 different countries has noted that exposure to childhood sexual abuse (CSA) could heighten the odds of later suicidal behavior, including suicide attempts. The study reported that those who experienced CSA before the age of 16 to 18 years had a two–fold increased risk of attempting or completing suicides.
  • A recent randomized trial has shown that diazepam is as safe and effective as lorazepam in treating children with status epilepticus (SE). Nearly 72.1% of those receiving diazepam and 72.9% of those receiving lorazepam met the primary efficacy endpoint of SE cessation within 10 minutes and without recurrence within 30 minutes, reported the study.

Why do we apply holy ash?

Bhasma is the holy ash produced from the Homa, the sacrificial fire, wherein special wood along with ghee and other herbs are offered as a part of pooja. By the time a Bhasma is formed, no trace of original matter remains in the ash. Ash obtained from any burnt object is not bhasma.

The ritual involves worshipping the deity by pouring ash as abhishek and then distributing it as Bhasma, which is then applied on the forehead (usually), upper arms, chest, or rubbed all over the body. Some consume a pinch of Bhasma when they receive it.

The word Bhasma is derived from bha or "bhartsanam" ("to destroy") and "sma" or "smaranam" ("to remember"). It denotes "that by which our sins are destroyed and the Lord is remembered". Bhasma is also called vibhuti, which means glory. Bhasma is associated with Lord Shiva who applies it all over His body.

Spiritually, the Homa is the offering of oblations into the fire with sacred chants and signifies offering or surrender of the ego and egocentric desires into the fire of knowledge. The resultant ash signifies the purity of the mind. The fire of knowledge burns the oblation and wood signifying ignorance and inertia respectively.

The application of ash implies that one should burn false identification with the body.

Bhasma has medicinal values in Ayurveda. It is supposed to be the strongest of all Ayurveda preparations. Ayurveda defines a Bhasma, when the matter is converted into non matter by the process of homa. The non matter is the spirit or the energy of the matter being processed with strong healing powers. It has the same significance as any ‘potentised’ medicine in homeopathy.

It absorbs excess moisture from the body and prevents colds and headaches.

When applied with a red spot at the centre, the mark symbolizes Shiva–Shakti (the unity of energy and matter that creates the entire seen and unseen universe).

The Upanishads say that the famous Mrityunjaya Mantra should be chanted whilst applying ash on the forehead.

Amiodarone linked to Cancer Risk in Men

The risk of cancer is increased in men taking amiodarone particularly in those with extensive exposure to the drug as per a retrospective study. Among men taking amiodarone, the risk for any cancer rose by almost 20% compared with the general population, according to Chia–Jen Liu, MD, of National Yang–Ming University Hospital in Yilan, Taiwan, and colleagues. And for men whose cumulative defined daily doses in a year exceeded 180, the risk was 46% higher the researchers reported online in Cancer.

The Man In Trouble

A man was caught and thrown into a pit (A big hole on earth) of about "40 feet" deep, just because he was a stranger. He struggled trying to get out of the pit,but the more he tried, the weaker his muscles became. He said to himself, "I don’t know how to get myself out of this. Maybe I should just die instead of enduring these miserable struggles and pains."

Just then, he heard a voice screaming, "Help! Help!! Help!!!" The shouts were coming from another pit, which was 10 feet more deeper than the one he was in.

He thought to himself, "Wow, so there is somebody else like me trapped here too". Listening carefully, he heard some cracking and sand dropping from the wall of the pit of the other victim. Immediately he summed up his courage and with his last bit of strength, started crawling little by little until he made it out of his pit.

MORAL:

That pain you think you are passing through, there are people worst off than you. Always say something sweet to yourself. Always smile at your pains. Let them be; they are just there for a while and also to challenge you. Today may be your darkest hour, but your joy will come in the morning. Try to encourage yourself. Look at yourself in the mirror, beat your chest and say, "I can make it and I can stand the test of time."

Summer Diarrhea Treatment

One loose motion is equal to one glass of ORS. Most summer diarrhea is watery, effortless and painless diarrhea with no mucous or blood. They need no antibiotics. The only treatment is oral rehydration solution, said Padma Shri, Dr. B C Roy National Awardee & DST National Science Communication Awardee, Dr. K K Aggarwal, President Heart Care Foundation of India and Sr National Vice President Indian Medical Association.

One loose motion is equivalent to loss of one glass of fluid. Explaining this, Dr. Aggarwal said that if a patient has passed 10 loose motions, he needs 12 glasses of ORS apart from normal requirement of fluids and one glass for every loose motion passed again.

For upto 12 loose motions a patient can be managed as an out patient but if the loose motions are more than 12 he needs observation and if the numbers of loose motions are more than 40, he needs intensive treatment.

The main complication is renal shut down, which will happen if the blood pressure remains low for more than few hours. During loose motions one should maintain that he or she passes urine every 6–8 hours. If no urine is passed for eight hours it may indicate an impending renal failure.
