Smell check, scratch and sniff, a new test for Parkinson’s disease

Olfactory dysfunction presenting as odor detection, discrimination, and identification is a common finding in patients with early non vascular Parkinson’s disease.

As per a study of 2,267 men published in the Annals of Neurology, an impaired sense of smell could be an early indicator of Parkinson’s disease, occurring up to four years before motor skill problems appear.

In the study decreased odor identification was associated with older age, smoking, more coffee consumption, less frequent bowel movements, lower cognitive function and excessive daytime sleepiness, but even after adjusting for these factors, those with the lowest odor identification scores had a five time greater risk of developing Parkinson's disease than those with the highest scores.

Nerve loss and the formation of Lewy bodies –– abnormal clumps of proteins inside nerve cells that are thought to be a marker of the disease –– are known to take place in the olfactory structures of patients with the disease.

An impaired sense of smell could also be caused by impaired sniffing, which may be another motor symptom of Parkinson’s.

Early indicators of Parkinson’s disease are olfactory abnormality, constipation and sleep disturbances.

Besides Parkinson’s disease, obesity, diabetes, hypertension, malnutrition, Alzheimer’s disease, multiple sclerosis and Korsakoff’s psychosis are all accompanied or signaled by smell disorders.

  • Ultrasound is equal to CT for emergency department evaluation of suspected kidney stones as shown in a randomized trial showed, said Marshall Stoller, MD, of the University of California San Francisco. Ultrasound performed by a radiologist or emergency physician led to serious adverse events in about 11% of cases, as did CT. Severe and attributable serious adverse events occurred in fewer than 1% of 2,800 patients and rates did not differ between patients evaluated by ultrasound or CT, Stoller and colleagues reported here at the American Urological Association meeting.
  • A crossover trial, published online May 15 in Diabetologia, has pointed that eating just 2 large meals a day, consisting of breakfast and lunch, could be the best way to control weight and blood sugar for people with type 2 diabetes. Patients following this regime lost more weight and had less hepatic fat and lower blood glucose, fasting insulin, and fasting glucagon than those consuming 6 smaller meals.
  • A novel research from the UK has revealed that in patients with type 1 diabetes who exercise in the evening, a low–glycemic meal and bedtime snack prevent hyperglycemia and early hypoglycemia; however they do not prevent late nocturnal hypoglycemia. The study was published online in Diabetes Care.
  • A study presented at the Digestive Disease Week (DDW) annual meeting has reported that MUSE™ System (less invasive surgical anterior fundoplication with standard surgical staples) is safe and effective for patients with PPI–responsive, moderate–to–severe GERD. Researchers noted that the majority of patients (74%) treated with the MUSE™ System remained off daily PPI at three years post–procedure.
  • The European Commission (EC) has cleared for marketing the protease inhibitor simeprevir for treatment of adults with hepatitis C (HCV) genotypes 1 and 4 infection in combination with other drugs. The approval is based on three phase III studies involving more than 1000 patients.

What precautions should be taken while vaccinating by the ID route?

  • The ID injections must be administered by staff trained in this technique. The Vaccine vials must be stored at +2°C to + 8°C after reconstitution.
  • The total content should be used as soon as possible, but at least within 8 hours.
  • The 0.1 ml. ID administration of cell–culture vaccine should create a wheal of at least 5 mm diameter with "peau de orange" appearance.
  • If ID dose is given subcutaneously then there is a possibility of poor immune response due to low antigen load. This may be life–threatening.

  • A study published online in Hypertension has shown that visit–to–visit variability in blood pressure (BP) is not associated with mortality in elderly primary care patients; however, long–term changes in BP are. Study authors noted that both large and small BP changes during follow–up were linked with higher mortality than moderate BP changes over time.
  • A study presented at the American Society of Hypertension 2014 Annual Scientific Meeting has revealed that BP measured in the clinic has no association with the risk of cardiovascular outcomes, including stroke. On the contrary, nighttime blood pressure may be associated with a risk of adverse clinical outcomes. Researchers noted a 25% increase in the risk of CV outcomes for every 10–mm–Hg increase in nighttime systolic BP.

  • An analysis published in the June issue of Pediatrics states that children with complex chronic conditions have increased likelihood to visit emergency departments (EDs) for drug reactions than other children. The highest rates of adverse events for these children are associated with drugs like psychotropic agents, antimicrobial agents, anticonvulsants, hormones, and analgesics.
  • Two separate articles being published in Annals of Internal Medicine have pointed that less frequent echocardiographic screening of childhood cancer survivors is effective for detecting asymptomatic left ventricular dysfunction (ALVD) and appears to be more cost–effective than following the widely accepted Children’s Oncology Group (COG) screening guidelines.

Best time to sign a deal is at 4 pm

As per Ayurveda 2–6 pm in the evening and 2–6 am in the morning are the periods of Vata or creativity. Most poets and writers do their creative work during these times of the day, especially 2–6 am in the morning.

Vata period is more creative and you are less likely to make mistakes. Four PM in the evening is considered the best time to make a deal, sign a document or to send a confession note.

Apart from the time of the day Vata age is old age and Vata month is rainy season. This is one reason that we always pay attention to the advice of the elderly.

A mix of exercise protocol is better

A combination of weight training and aerobic exercise is the best prescription for overweight patients at risk for diabetes and heart disease. Only aerobic exercise is also as good as it reduces weight and takes inches off the waistlines. Just weight lifting alone has very little benefit.

According to a study published in the American Journal of Cardiology, people in the weight–training group gained about 1.5 pounds and those in the aerobic group lost an average of 3 pounds and half an inch from their waists.

Those who did both weight and aerobic training dropped about 4 pounds and 1 inch from the waistline. This group also saw a decrease in diastolic lower blood pressure and in a metabolic syndrome score.

Both the aerobic–only group and the combined-exercise group also lowered their levels of bad triglycerides.

Once upon a time the colors of the world started to quarrel. All claimed that they were the best…the most important, the most useful, the most beautiful and the favorite.

Green said: "Clearly I am the most important. I am the sign of life and of hope. I was chosen for grass, trees and leaves. Without me, all animals would die. Look over the countryside and you will see that I am in the majority."

Blue interrupted: "You only think about the earth, but consider the sky and the sea. It is the water that is the basis of life and drawn up by the clouds from the deep sea. The sky gives space and peace and serenity. Without my peace, you would all be nothing."

Yellow chuckled: "You are all so serious. I bring laughter, gaiety, and warmth into the world. The sun is yellow, the moon is yellow, and the stars are yellow. Every time you look at a sunflower, the whole world starts to smile. Without me there would be no fun."

Red could stand it no longer he shouted out: "I am the ruler of all of you. I am blood – life’s blood! I am the color of danger and of bravery. I am willing to fight for a cause. Finally Indigo spoke, much more quietly than all the others. But with just as much determination: "Think of me. I am the color of silence. You hardly notice me, but without me you all become superficial. I represent thought and reflection, twilight and deep water. You need me for balance and contrast, for prayer and inner peace."

And so all the colors went on boasting each other. Each convinced of his or her own superiority. Their quarreling became louder and louder. Suddenly there was a startling flash of bright lightening, thunder rolled and boomed. Rain started to pour down relentlessly. The colors crouched down in fear, drawing close to one another for comfort.

In the midst of the clamor, the Rain began to speak: "You foolish colors, fighting amongst yourselves, each trying to dominate the rest. Don't you know that you were each made for a special purpose, unique and different? Join hands with one another and come to me."

Doing as they were told, the colors united and joined hands.

The Rain continued, "From now on, when it rains, each of you will stretch across the sky. In a great bow of color as a reminder that you can all live in peace. The Rainbow is a sign of hope for tomorrow."

And so, whenever a good rain–washes the world and a Rainbow appears in the sky, is to let us remember to appreciate one another.

Porcine Whipworm Ova Safe for Treatment of IBD

Orally ingested ova of Trichuris suis, the porcine whipworm, has been found to be active in an open–label study of Crohn’s disease, and in a small scale, placebo–controlled trial in patients with ulcerative colitis, said Padma Shri, Dr. B C Roy National Awardee & DST National Science Communication Awardee, Dr. K K Aggarwal, President Heart Care Foundation of India and Sr National Vice President Indian Medical Association.

Inflammatory bowel diseases are diseases of the affluent society. Many trials have shown that infection with parasitic worms is protective. Intestinal helminthes induce Th2 cytokine release and specifically down regulate Th1 responsiveness

Administering eggs from the porcine whipworm Trichuris suis to patients with Crohn’s disease and ulcerative colitis is a safe and possibly effective treatment for inflammatory bowel disease according to a report published in the American Journal of Gastroenterology. In the open–label trial, 29 patients with refractory disease, and a Crohn’s disease activity index (CDAI) of 220 to 450, ingested 2,500 T. suis ova in a beverage every three weeks for 24 weeks. By week 12, 22 patients (75.9%) experienced a decrease in CDAI of more than 100 points, or had a CDAI of less than 150. Another 18 patients (62.1%) were in remission. (Four patients withdrew early because of disease activity or pregnancy.) They observed no adverse effects or complications. In the treatment a single dose of 2500 live Trichuris suis eggs are given orally.
