Water Hygiene

Safe water is an essential commodity for prevention of most water and food-borne diseases like diarrhea, typhoid and jaundice. These diseases are 100% preventable. All of them can be lethal if not prevented, diagnosed or treated in time.

Transmission of parasitic infections can also occur with contaminated water. Here are a few tips: V7

  • Travelers should avoid consuming tap water.
  • Avoid ice made from tap water.
  • Avoid any food rinsed in tap water.
  • Chlorination kills most bacterial and viral pathogens.
  • Chlorination does not kill giardia cysts.
  • Chlorination does not kill amoeba cysts.
  • Chlorination does not kill cryptosporidium.
  • Boiled water is safe.
  • Treated water is safe.
  • Bottled water is safe.
  • Carbonated drinks, wine and drinks made with boiled water are safe.
  • Freezing does not kill the organisms that cause diarrhea. Ice in drinks is not safe unless it has been made from adequately boiled or filtered water.
  • Alcohol does not sterilize water or the ice. Mixed drinks may still be contaminated.
  • Hot tea and coffee are the best alternates to boiled water.
  • Bottled drinks should be requested without ice and should be drunk from the bottle with a straw rather than with a glass.
  • Boiling water for 3 minutes followed by cooling to room temperature will kill bacterial parasites.
  • Adding two drops of 5% sodium hydrochloride (bleach) to quarter of water (1 liter) will kill most bacteria in 30 minutes.
  • Adding five drops of tincture of iodine to a quarter of water (1 liter) will kill bacteria within 30 minutes.

  • A study presented at the European Association of Urology (EAU) 29th Annual Congress has revealed that men with moderate symptoms of benign prostate hyperplasia when treated with dutasteride plus tamsulosin hydrochloride present with better outcomes than when they are monitored with watchful waiting. Mean change in International Prostate Symptom Score (IPSS) was significantly greater in the treatment group than in the watchful waiting group (5.4 vs 3.6), reported the study.
  • Two parallel clinical trials have stated that patients with neovascular age–related macular degeneration maintained on a stable proactive regimen of quarterly intravitreal injections with the vascular endothelial growth–factor (VEGF) inhibitor aflibercept have more stable visual outcomes than patients treated on a reactive as–needed basis. The findings were presented recently at the World Ophthalmology Congress (WOC) 2014.
  • An article published online April 16 in BJOG stated that in most women, the menopausal transition is characterized by at least 3 occurrences of menses for 10+ days (77.7% women) and spotting for 6+ days. Black women are less likely than white women to report this pattern. Additionally, education, economic strain, and body mass index may not be associated with such a pattern.
  • Addition of pegylated interferon to nucleos(t)ide analog therapy improves viral decline and response rates in patients with hepatitis B e antigen-positive chronic hepatitis B, reports a global randomized trial. The hepatitis B e antigen loss with HBV DNA below 200 IU/mL at week 48 was achieved by 19% patients in the add–on group vs. 10% in the monotherapy group. The trial was presented at the European Association for the Study of the Liver (EASL) International Liver Congress 2014.
  • A novel study conducted at the North Texas Health Science Center has reported that while endogenous testosterone may be beneficial under conditions of low oxidative stress, it may have negative effects over cognition under circumstances of high oxidative stress, although only in white men. The study authors therefore suggested that older white men considering testosterone replacement therapy (TRT) should first have homocysteine levels checked to determine their oxidative stress status.

Can rabies vaccine be given to a child with chicken pox or measles?

As rabies is 100% fatal, there is no contraindication for antirabies vaccination. Rabies vaccine can be given to a child with chicken pox or measles and it is effective. If possible administration of measles vaccine should be postponed by a fortnight after the completion of antirabies immunization.

  • US health regulators have recently approved four Boston Scientific Corp implantable devices for heart patients, including a next generation of smaller defibrillators to treat abnormal heart rhythms. The devices include Dynagen Mini and Inogen Mini implantable cardioverter defibrillators (ICDs) and the Dynagen X4 and Inogen X4 cardiac resynchronization therapy defibrillators (CRT–Ds).
  • Obtaining an ECG in the prehospital setting increases the odds of patients with ST–elevation MI (STEMI) getting reperfusion therapy by 70%, reports a recent cohort study based on registry data from England and Wales. It can also significantly improve 30–day survival in both STEMI and non–ST–elevation MI (NSTEMI) patients. The study was published April 14 in Heart.

  • A large number of patients with autism spectrum disorder (ASD) have evidence of mitochondrial dysfunction on magnetic resonance studies, reports a new research in JAMA Psychiatry. Lactate doublets, an indication of mitochondrial dysfunction, were present in significantly more individuals with ASD (13%) than in typically developing controls (1%).
  • A new study conducted in Canada has found that teenagers who have suffered a traumatic brain injury, such as concussion, have two–fold increased odds of being victims of school bullying or cyber–bullying, and are also three times more likely to attempt suicide or be threatened with a weapon at school compared to those who have never suffered such an injury.

In legal system when you are not satisfied with a judge, you re–appeal in the same court and if you are still not satisfied, you go to double bench before going to higher court.

Practice the same when you take a second opinion about your health.

Never go to a different doctor. First go to the same doctor and ask him to give his opinion again and, if you are not satisfied, then go to a team of two doctors and finally go to a doctor with qualification higher than the initial doctor. If your second opinion is from another doctor, it may invariably be wrong or partial.

Depression Should Be Treated In Patients with Diabetes

Presence of depression in diabetic patients is the most important factor leading to erectile dysfunction in such patients.

An India cum US study published in the Journal of Urology, depression and erectile dysfunction are related to each other in the form of a vicious cycle. Depression instigates erectile dysfunction and erectile dysfunction perpetuates the symptoms of depression.

  1. India is the diabetic capital of the world and both the incidence and prevalence of diabetes are increasing day by day.
  2. It is, therefore, important for the family physician to look for presence of depression in patients with diabetes.
  3. Diabetic patients who complain of erectile dysfunction in the study also have higher prevalence of high blood pressure and cholesterol abnormalities. These patients were also of older age with uncontrolled diabetes, history of smoking and longer duration of diabetes.

It was also shown that patients who develop erectile dysfunction also had diabetes, eye disorders, neuropathy and peripheral vascular disease.

Effective control of diabetes may reduce both depressive symptoms and erectile dysfunctions in such cases.

The house with the golden windows

The little girl lived in a small, very simple, poor house on a hill and as she grew she would play in the small garden and as she grew she was able to see over the garden fence and across the valley to a wonderful house high on the hill – and this house had golden windows, so golden and shining that the little girl would dream of how magic it would be to grow up and live in a house with golden windows instead of an ordinary house like hers.

And although she loved her parents and her family, she yearned to live in such a golden house and dreamed all day about how wonderful and exciting it must feel to live there.

When she got to an age where she gained enough skill and sensibility to go outside her garden fence, she asked her mother is she could go for a bike ride outside the gate and down the lane.

After pleading with her, her mother finally allowed her to go, insisting that she kept close to the house and didn’t wander too far. The day was beautiful and the little girl knew exactly where she was heading! Down the lane and across the valley, she rode her bike until she got to the gate of the golden house across on the other hill.

As she dismounted her bike and lent it against the gate post, she focused on the path that lead to the house and then on the house itself…and was so disappointed as she realized all the windows were plain and rather dirty, reflecting nothing other than the sad neglect of the house that stood derelict.

So sad she didn’t go any further and turned, heart broken as she remounted her bike … As she glanced up she saw a sight to amaze her…there across the way on her side of the valley was a little house and its windows glistened golden …as the sun shone on her little home.

She realized that she had been living in her golden house and all the love and care she found there was what made her home the ‘golden house’. Everything she dreamed was right there in front of her nose!

Deep frying, burning or charring foods may produce cancers

Regularly eating burned or charred red meat, similar to that cooked on a grill can increase the chances of pancreatic cancer by 60%, said Padma Shri, Dr. B C Roy National Awardee & DST National Science Communication Awardee, Dr. K K Aggarwal, President Heart Care Foundation of India and Sr National Vice President Indian Medical Association.

Turning down the heat when grilling, frying, and barbecuing to avoid excess burning or charring of the meat may be a sensible way for some people to lower their risk for getting pancreatic cancer.

As per Dr Kristin Anderson of the University of Minnesota, charred meat contains several known cancer-causing chemicals, including heterocyclic amines.

Heterocyclic amines (HAs) are carcinogens produced by cooking meat and animal protein at high temperatures; metabolism of HA is influenced by polymorphisms in the N–acetyltransferase–2 (NAT–2) gene. Data from a variety of sources suggest that HA may play a role in human carcinogenesis and are liked to many cancers.

Anderson's team started with 62,000 healthy people and documented what they actually did eat. Over nine years, 208 were diagnosed with pancreatic cancer. When divided into five groups based on how much charred meat such as hamburgers they ate, the people diagnosed with pancreatic cancer were far more likely to be in the top two groups.

They found that those who preferred very well-done steak were almost 60 percent more likely to get pancreatic cancer as those who ate steak less well-done or did not eat steak. Those with the highest intake of very well–done meat had a 70 percent higher risk for pancreatic cancer over those with the lowest consumption.

The mutagenic activity and the mass amount of heterocyclic amines responsible for the mutagenic activity have been measured in some cooked foods. Cooked meats are the predominant source of mutagenic activity in the diet with values ranging from 0 to 10,000 revertants per gram reported in the Ames/Salmonella test with strain TA98.

Frying at higher temperatures and for longer times produces the greatest mutagenic response, and thus concomitantly, the largest amounts of heterocyclic amines.

Non–meat products such as baked breads can also form significant mutagenic activity, particularly when overcooked. Commercially prepared hamburgers made from meat substitutes such as tofu, wheat gluten or tempeh and fried at 210 degrees C have up to 10% of the mutagenic activity of a fried beef patty cooked under the same conditions.

Lesson: Make Sure to perform an emergency diagnostic angiography and mechanical revascularization with PTCA in patients of cardiogenic shock. Results of NRMI–2, an ongoing trial suggest that this intervention is much better than thrombolytic therapy in such patients.
