To control TB, prevent diabetes in the community

If you want to control TB, prevent diabetes in the community especially in India where rates of TB are amongst the highest and where the incidence of type 2 diabetes is rising sharply.

As per a WHO report published online September 4 in Lancet Diabetes & Endocrinology, patients with either diabetes or TB need to be cross-checked for the other disease.

Fifteen percent of adult TB cases worldwide are already attributable to diabetes, with more than 40% occurring in India and China alone.

Diabetes can go undiagnosed for a long period, so it makes sense to do proactive screening for diabetes in all patients with TB.

Conversely, diabetes should be on the clinical radar when caring for people with TB.

News Around The Globe

  • Prediabetes, even at lower levels of impaired fasting glucose (IFG) as defined by the American Diabetes Association (ADA), is associated with a considerable increase in cancer risk that is independent of the effects of obesity, suggests a large new meta–analysis published online September 8 in Diabetologia. The risk may be increased particularly for liver and stomach or colorectal cancer, suggested the study.
  • A new study presented at the PAINWeek 2014 has revealed that patients with fibromyalgia can safely be treated with the anticonvulsant drug pregabalin for pain while being treated for depression simultaneously. Study authors noted that pregabalin significantly improved mean pain scores in patients with fibromyalgia taking a selective serotonin reuptake inhibitor (SSRI) or a serotonin/norepinephrine reuptake inhibitor (SNRI).
  • A preliminary study of exhaled breath has pointed that temperatures higher than 34°C may strongly indicate the presence of lung cancer. The study was presented at the European Respiratory Society (ERS) International Congress 2014.
  • A tailored approach can reduce anti–TNF drug doses while apparently maintaining efficacy in patients with ankylosing spondylitis (AS), suggested a new study published online in the Annals of the Rheumatic Diseases. Study authors advocate that all patients with AS do not need the same level of TNF inhibition, and many of them can do well on substantially lower doses of anti–TNF drugs.
  • A novel study, published online September 9 in BMJ, noted that the use of benzodiazepines to treat insomnia or anxiety may increase the risk for Alzheimer’s disease (AD). The risk for AD was increased by 43% to 51% in those who had ever used benzodiazepines in the previous 5 years, reported the study.

  • The International Fixed Dose Combination Drug for Secondary Cardiovascular Prevention (FOCUS) trial of patients who had survived an MI showed that taking a fixed–dose combination aspirin/simvastatin/ramipril polypill increases the odds of perfect pill compliance; however, only about half of the patients were consistently taking the polypill at the nine–month study end. The data were presented at the European Society of Cardiology (ESC) 2014 Congress.
  • A new proof–of–concept study has demonstrated that contactless video monitoring can identify changes in skin color in the human face during atrial fibrillation (AF). Detection of these beat-to-beat variations in skin color resulting from changes in blood flow with the cardiac pulsatile signal helped identify episodes of AF with just a few seconds of recorded video. The study was published recently in Heart Rhythm.

Pediatrics eMedinewS

  • New research has shown that an intervention that helps parents more successfully engage with their infants can reverse marked symptoms of autism and the usual pattern of developmental deceleration by the time infants reach the age of 36 months. Researchers noted that infants who received a parent–implemented interactive program exhibited normal patterns of both verbal and nonverbal development. The study was published September 9 in the Journal of Autism and Development Disorders.
  • A new study has found that factors, such as birth weight, gestational age at birth and lung function, growth and other measures at 8 years, can successfully predict lung function during mid to late teenage years. The study was presented at the European Respiratory Society (ERS) International Congress.

Do Not Tell Others, It’s A Secret?

Most Asians are known for two habits, firstly a ‘crab’ nature and secondly the habit of telling others that "I am telling you a secret and please do not tell others."

Crab nature mean pulling each other’s legs and not allowing any one to rise. Most people, especially women, will gossip and tell all secrets that they know. For clearing their own consciousness they will add the sentence that” I am telling you the secret and do not tell others. The very fact you are telling it to someone it is no more a secret. The recent case of Rajat Gupta in the US is an extension of the same.

In India we do not respect confidentiality laws and do not sign a non disclosure agreement. In hospitals you will hear the names of the hospitalized person being shouted loud outside the ICU; the list of operation theatre is displayed at every corner of the hospital.

Pumpkin Extract Beneficial for Diabetic Patients

Pumpkin extract has insulin–like effects. It can help people with diabetes keep their blood sugar under control.

Quoting Chinese researchers that animals with drug–induced diabetes treated with pumpkin extract had lower blood glucose levels, greater insulin secretion, and more insulin–producing beta cells than diabetic rats that weren’t given the extract. This action may be due to the presence of both antioxidants and D–chiro–inositol, a molecule that mediates insulin activity.

Pumpkin extract is potentially a very good product for pre–diabetic persons, as well as those who have already developed diabetes.

Pumpkin is frequently used to treat diabetes and high blood glucose in Asia.

The results of an animal study have shown that rats with diabetes had 41 percent less insulin in their blood than normal rates; giving them pumpkin extract for 30 days boosted levels of the blood sugar–regulating hormone by 36 percent. And after 30 days of being fed pumpkin extract, diabetic rats had blood glucose levels similar to those of non-diabetic rats.

Love Wealth and Success?

A woman came out of her house and saw three old men with long white beards sitting in her front yard. She did not recognize them. She said "I don’t think I know you, but you must be hungry. Please come in and have something to eat."

"Is the man of the house home?" they asked. "No", she said. "He’s out." "Then we cannot come in", they replied. In the evening when her husband came home, she told him what had happened. "Go tell them I am home and invite them in!" The woman went out and invited the men in.

"We do not go into a House together," they replied. "Why is that?" she wanted to know. One of the old men explained: "His name is Wealth," he said pointing to one of his friends, and said pointing to another one, "He is Success, and I am Love." Then he added, "Now go in and discuss with your husband which one of us you want in your home."

The woman went in and told her husband what was said. Her husband was overjoyed. "How nice!!", he said. "Since that is the case, let us invite Wealth. Let him come and fill our home with wealth!" His wife disagreed. "My dear, why don't we invite Success?"

Their daughter–in–law was listening from the other corner of the house. She jumped in with her own suggestion: "Would it not be better to invite Love? Our home will then be filled with love!"

"Let us agree to our daughter–in–law’s advice," said the husband to his wife. "Go out and invite Love to be our guest."

The woman went out and asked the three old men, "Which one of you is Love? Please come in and be our guest." Love got up and started walking toward the house. The other 2 also got up and followed him.

Surprised, the lady asked Wealth and Success: "I only invited Love. Why are you coming in?" The old men replied together: "If you had invited Wealth or Success. The other two of us would've stayed out, but since you invited Love. Wherever He goes, we go with him.

Wherever there is Love, there is also Wealth and Success!!!!!!"

Heart Care Foundation of India and Friends of Vrindavan launch Project Vrindavan

An initiative under the Sameer Malik Heart Care Foundation Fund to ensure that no person in Vrindavan dies of heart disease because they cannot afford treatment

Vrindavan, September 10th, 2014 Heart Care Foundation of India, a leading national non–profit organization committed to making India a healthier and disease free nation and Friends of Vrindavan, jointly announced the launch of Project Vrindavan. An initiative under the Sameer Malik Heart Care Foundation Fund, a project by the Heart Care Foundation of India; Project Vrindavan is aimed at providing technical and financial support to all those people in Mathura who belong to economically weaker sections of the society and cannot afford treatment for heart disease. A spiritual workshop by the NGO’s President and National Awardee, Dr. KK Aggarwal was organized to mark the commencement of the project. Archarya Shri Vats Goswami, the Head priest of Radha Raman Mandir in Vrindavan and the founder of Friends of Vrindavan welcomed several spiritual leaders to join and support the cause.

The underlying concept behind "Project Vrindavan" is to work towards eliminating blue babies from Vrindavan. Medically, a child suffering from cyanotic congenital heart disease is termed a blue baby. However, in the holy town of Vrindavan the color blue is worshipped as a representation of Lord Krishna depicting consciousness and vastness. Then why must children in Vrindavan succumb to heart disease just because they cannot afford treatment? Through Project Vrindavan, all people suffering from heart ailments, who otherwise don’t have the means to undergo surgery, will be provided assistance through the Sameer Malik Heart Care Foundation Fund. All surgeries will be done in leading hospitals in New Delhi and stay for patients will be provided at concessional rates.

Speaking on the occasion, Padma Shri, Dr BC Roy National & DST National Science Communication Awardee, and the President of Heart Care Foundation of India, Dr KK Aggarwal, said: "We at Heart Care Foundation of India are committed to raising awareness and finding effective solutions to the growing medical problems in the country. As a part of this endeavor, we launched the Sameer Malik Heart Care Foundation Fund in April this year with an aim to ensure that no person dies of heart disease just because they cannot afford treatment. Over the past few months we have helped save over 150 lives and our aim is to take this number to 500 by the end of this year. I am extremely happy to have Friends of Vrindavan and Shri Vats Ji join us in this initiative and bring this project to the holy city of Vrindavan. I hope that through Project Vrindavan, we can give a new life to all children suffering from a heart disease in Vrindavan and are unable to get treatment because of their economical background".

Adding to this, Archarya Shri Vats Goswami, Head priest of Radha Raman Mandir Vrindavan said, "We at Friends of Vrindavan are extremely happy to join hands with the Heart Care Foundation of India to announce the launch an initiative as noble in cause of Project Vrindavan. We hope that through our combined efforts, we can cure all blue babies in Vrindavan and help save as many lives as possible. I urge everyone to come and support the cause and help us make Vrindavan and healthier and disease free city.

Any person can apply for the benefits of the Sameer Malik Heart Care Foundation Fund by calling on its helpline number +919958771177 from Monday through Saturday from 9 AM to 5 PM. Individuals who wish to apply online can download the application form from the website, http://heartcarefoundationfund.heartcarefoundation.org/

Speaking about the initiative, Dr. Pawan Aggarwal,– Secretary Indian Medical Association Vrindavan branch and Jagannath Poddar, Director – Friends of Vrindavan in a joint statement said “It is our privilege to be associated with such a humane cause. We hope that together we can help not just the people in Vrindavan but all across India. We truly believe that every person has the right to live a healthy and happy life irrespective of his or her religion, caste, gender or economical condition".

An expert committee assesses all applications received by the Sameer Malik Heart Care Foudnation Fund. Once sanctioned, the funds are directly deposited in the bank account of the medical establishments treating the patient. Heart Care Foundation of India has also negotiated minimal rates with leading hospitals in the capital city for surgical treatments.
